Video Tutorial: Additional Family Information Form IMM5406


This form is used in many applications, from sponsorships to federal skilled worker applications and others.

Although this form is a short one-pager, from my experience, it causes endless grief for applicants as it is very easy to make an error while completing this form.

This video will highlight the difficulties of the form and provide some guidance in completing it. You should not rely on this video to ensure it is completed correctly however. You should complete the form in conjunction with Citizenship and Immigration Canada’s guides and instructions.

As well, forms are sometimes updated and this form may not match the current form.

    Could anyone help me understand on how my children will fill up this IMM 5406 Form? I thought this form only apply to the principal applicant but, on the Documents list, it say’s ,each of the family 18 years and over should fill up and signed by the person concerned? Please help? Thank you in advance.

    Hi – what sort of application is it? The principal applicant yes, and others applying for immigration over 18 years old. They would each be the “applicant” when completing the form. Hope this helps.
